Group Clinics Training and Support Programme
Overview
The Staffordshire Training Hub is supporting Practices and PCNs to deliver Group Clinics through a fully funded training and support programme delivered by Experienced Led Care (ELC).
What are Group Clinics
Group Clinics bring patients with similar conditions – such as asthma, hypertension and diabetes – together in a supportive, collaborative setting.
Patients receive planned clinical care from healthcare professionals while also learning from each other and benefiting from peer support.
Group Clinics help Practices or PCNs bring chronic disease management reviews back on track and improve QOF performance.
