Nurse Roots: Student Nurse Placement Confirmation

The Nurse Roots placement model has been developed to reduce pressure on practices/PCNs while providing students with a high-quality, holistic learning experience in primary care which ultimately supports the future primary care workforce. Completion of the below form will help inform the Staffordshire Training Hub of your availability to support pre-registered student nurses within your practice/PCN. Please let us know your preferences and availability so that we can facilitate appropriate student allocation. Please note that students on this hybrid placement typically spend 2.5 days in practice and are not in practice full time.

Information of the main areas of proficiencies are summarised below:

Episode of Care

The student will be given the opportunity to supervise and teach a junior learner/colleague in practice and provide a written reflection on this experience [e.g. inhaler technique/peak flow]. This needs to be based on the delivery of direct person-centred care. Professionalism underpins all aspects of the students’ performance.

Medicines Management

The student safely administers medicines to a group of patients/service users or a caseload of patients/service users

Professional Values

Students are required to demonstrate high standards of professional conduct at all times during their placements. Students should work within ethical and legal frameworks and be able to articulate the underpinning values of The Code (NMC, 2015). Professional Values reflect a number of proficiency statements and are captured under the 4 sections of The Code.
